The Care of the Elderly Team (SWFT) has a GP advice line to support the management of frail elderly people.

Single Point of Access for Community and Integrated Health


ISPA is a central point of access for referrals to the Integrated Health Teams throughout Warwickshire. If the call is non-urgent the referral can be emailed to the local ISPA.

The following teams can be accessed through iSPA:


1) Community Nursing and Long Term Condition Management

  • Community nurses undertake assessments, wound care, palliative and terminal care, LTC support and advice for patients who are housebound.

2) Community Emergency Response Team (CERT)

  • CERT provides support to patients on a time-limited basis and refer on to appropriate services as required.
  • They facilitate hospital discharges but can also be accessed by GPs to avoid an admission by providing timely support for patients to stay at home.
  • The team consists of therapists, nurses and support workers.

3) Intermediate Care (ICT)

  • ICT provide up to 6 weeks support to patients who have rehabilitation needs through a team of therapistis, nurses and support workers.


NOTE: A reablement service is provided by social care for patients who require a period of further support to achieve their optimum level.  It is usually a maximum of 6 weeks.



Macmillan Nurses

Macmillan nurses provide both palliative and terminal advice and support.

Falls services

South Warwickshire Specialist Falls Prevention Service

The team provide falls risk assessment (non-medical) for older people at risk of falling, therapeutic interventions including strength and balance exercises, and education to reduce risks of falling.

Age UK – Home Safety Check Service


A free Home Safety Check looks at your home through a fresh pair of eyes and advises you on improvements and fittings to make your home safer and life easier. A visual check of other potential safety hazards, especially fire, gas and electricity is included.

Equipment and home adaptations


Warwickshire Wheelchair Service                

The Warwickshire Wheelchair service provides people with long term substantial and permanent postural and mobility needs in their home environment with a prescribed wheelchair and accessories e.g. headrest, back support etc. and a repair/maintenance service.

Social care equipment and home adaptations

The service helps people to live in their own homes more easily by providing a range of equipment. The equipment can range from a simple walking stick, to a more complex item like a mobile hoist. To receive this service you must be assessed and eligible for social care support.
